Utilization Management and Case Management

The purpose of Utilization Management and Case Management is to monitor the quality and appropriateness of care provided to our patients and to assist them with discharge planning. Utilization Management communicates with insurance companies, Medicare, and Medicaid so that those entities know that our patients are being taken care of properly and are receiving all the benefits due them.

We constantly strive to be proactive when it comes to the health care that's being provided to our patients. Utilization Management also monitors all deaths within the facility and coordinates the Organ Donation Procurement program at St. John's Hospital - Lebanon.

Several projects are in place or are being developed to help educate patients about their specific conditions and prepare them for these life changing situations. Currently, Utilization Management offers assistance through the following Projects:

Congestive Heart
Failure (CHF) Project


With this program, our patients receive extensive information about CHF (including diet, exercise, help on how to stop smoking, basic "dos and don’ts," and medication), a free scale, a BP monitor, and Case Management will follow up with the patient for 6 weeks (or longer if the patient requests it), and constant feedback from us and the patient's physician.
Ostomy
(Colostomy) Project


With this program, the patient receives extensive information about his/her ostomy (including diet, care, equipment, and emotional support), a free ostomy kit, and Case Management will follow up with the patient for 6 weeks (or longer if the patient requests it), and constant feedback from us and the patient's physician.

In the future, the following Projects will be in place to assist our patients:

  • Stroke - CVA (Cerebral Vascular Accident)
  • Chronic Obstructive Pulmonary Disease (COPD)

Their normal hours of operation are Monday - Friday, 6:30 am - 3:00 pm, and they are on call 24 hours a day, 7 days a week.
